Current Open Positions 

Indigenous Housing First Case Manager

Facilitate intensive case management to support households through individual case management with the goal of maintaining housing and achieving optimum quality of life.  Case management includes:  complete program intakes when a client is assigned, completing housing plans, goal plans, substance use assessment forms, crisis planning, safety planning, etc.

 

To provide and enhance life skills, addressing health and mental health needs, engaging in meaningful activities and building social and community relations, including transportation for basic needs, appointments, viewing, etc.  Complete monthly budgets for housed clients including unit inspections. Assisting in completing applications, gathering documents (including identification), attending landlord interviews or housing viewings, and assisting with processes related to lease signings.

 

Household set up, counselling, access to traditional healers, ceremonies and medicines, and other activities related to improved health and well-being.

Indigenous Housing  Support Worker 

This position will provide direct support to the Indigenous Housing First Case Manager(s).  This will include:  weekly housing searches for unhoused clients, locating persons who have been prioritized through the Simcoe County Coordinated Access/By-Name list.  Completing intakes with client who agree to sign with the program.  Assist housing first case managers with housing plans.   

 

Assisting in completing applications, gathering documents (including identification), attending landlord interviews or housing viewings, and assisting with processes related to lease signings.

Indigenous Program Support Worker 

The position supports clients residing in our transitional housing program including outreach support to clients who have successfully transitioned into permanent housing. 

Individual support and transportation to clients of the transitional housing program and housing first program as necessary, support client goal plans and assist with follow through of client requests, referrals, etc., identified in the goal plans

Nutrition shopping for meal program of the residential program

Assist with daily meal preparation for the residential program

Complete orientation with new clients moving into residential program, assist with client move in and clients exiting the program (move out)

Complete weekly unit inspections for the residential program

Purchase and restock house supplies as necessary for the residential program, complete weekly inventory list

Complete crisis planning tools for the new clients moving into the residential program

Complete external referrals as necessary for the clients of the residential program

Supervision of clients residing within the transition homes, ensuring safety and security measures are met; as well as, compliance with policies

Respond to crisis situations and or emergencies as necessary

Cleaning as required of the communal living home and or individual units of the residential program

Complete weekly and monthly reports as required; participate in team meetings and training
